Our team partners with you, your family, and your physician to ensure that you receive personalized quality care. Patients heal faster at home but, without adequate support, could end up back in the hospital soon after discharge. Our various clinical specialties provide a wide range of care to allow patients with medical conditions to stay at home.

Nursing Care

Our nursing staff help patients recover at home after an injury, illness, surgery, or complications related to chronic conditions. In-home visits assess patients' current condition and provide appropriate treatments and teaching in collaboration with their physicians, to support and encourage independence in their homes.

Rehabilitation

VNH offers Physical, Occupational, and Speech Therapy to support rehabilitation following surgery, stroke, injury or illness, and support independence in the days and months ahead.

Physical Therapists assess each patient and develop an individualized plan of care. They are movement experts who optimize quality of life through teaching and treatment techniques that benefit long term health.
Treatments may include:

  • Therapeutic exercises to improve strength, joint range of motion, balance, endurance and general reconditioning
  • Individualized home exercise programs
  • Bed mobility and transfer training
  • Necessary assistive devices or medical equipment
  • Massage to decrease pain, manage scar tissue, muscle spasm and/or edema, and improve flexibility
  • Patient education, teaching and support
  • Home safety and fall prevention evaluation
  • Applications such as ultrasound, heat packs and ice packs as appropriate

Occupational Therapists assess home safety and accessibility, recommending adaptive equipment and home modifications to maximize independence. They also teach patients how to accomplish the basic activities of daily living.
Services may include:

  • Instruction and training on dressing, bathing, and other activities
  • Strengthening of upper body and trunk
  • Assessment and evaluation of fine motor activity and treatment to improve function.

Speech Language Pathologists provide comprehensive assessment and care for a variety of speech, cognition, and swallowing difficulties.

Home Health Aides

Working under the direction of VNH nurses, Licensed Nursing Assistants (LNAs) help those in our care with basic health needs such as taking vital signs and obtaining specimens, as well as activities of daily living including moving from bed, dressing, eating, and bathing.

Medical Social Work

In addition to medical care, social workers provide support to patient, family, and caregivers and facilitate access to community resources as needed to help them manage the challenges associated with illness or the care of a loved one.

Questions about home healthcare

Home health care is available for people of all ages. This may include those who have been discharged from an inpatient facility, have been referred by their physician and have short term health needs, or who need assistance due to age or disability to remain home.

Many people are eligible for home care after a hospital stay in order to effectively heal and recover at home. If you think you may be eligible, contact us for more information.

Medicare and many other insurance providers cover all or a portion of care provided by certified home health care agencies, such as VNH. Check with your individual insurance carriers for your benefits.

A patient, family member, or physician can request service. However, Medicare, Medicaid, and most insurance plans require a physician's referral for certified home health care.
