We provide an initial assessment to help identify the nature of your child's needs, the impact on your family, and the goals for hospitalization.
We work closely with you and pre-hospitalization treatment providers to identify aftercare needs early in hospitalization. Social workers work with you, your child, and the outpatient teams to coordinate aftercare services and a successful transition at the time of discharge.
